Summary/Objective
The Licensed Practical Nurse (LPN) is a valued member of the care team and provide services to meet the needs of individuals and families at Hospice House. Under the supervision of an RN, the LPN provides direct nursing care, within the scope of their license, to patients as directed by the physicians and other healthcare providers. Cooperative decision-making is expected in the execution of duties.
Essential Functions:
- Implements and evaluates patient care under the supervision of an RN. Participates in the development of a patient-specific care plan.
- Follows plan of care, under the supervision of physician and RN case manager.
- Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition,
- Provides direct patient care including administering of medications and treatments as prescribed.
- Teaches and counsels the patient and family/significant others regarding nursing care needs according to the plan of care.
- Regularly reevaluates the patient’s nursing needs, communicating any changes in need or condition to the physician, RN and other necessary clinical staff.
- Maintains accurate and complete records of observations, treatments and care of patient and documents patient care in electronic medical record according to department standards.
- Delegates, prioritizes, requests and accepts guidance and assistance from others to ensure delivery of care in a safe and timely manner.
- Makes nursing visits within the patient’s private home and facilities.
- Understands and adheres to established policies and procedures.
- Attends staff meetings, patient care conferences and in-services as scheduled.
- Participates in patient care conferences/IDT as directed by the Supervisor.
- Assists the RN in performing specialized procedures as directed.
- Ensures own professional effectiveness through continued education and professional development.
- Completes other duties as assigned.

Required Skills & Qualifications:
- Graduate of an accredited school of nursing with unencumbered licensure to practice as a Licensed Practical Nurse in the State of Oregon.
- Minimum of 1-year recent experience in a clinical setting; home health/ hospice care experience preferred.
- At least 18 years of age.
- Ability to pass reference checks, background test, drug screen, and TB test.
- Valid CPR Certification.
- Excellent assessment, problem-solving skills, nursing skills per competency checklist.
- Strong clinical judgment and patient triaging skills are vital; efficient time management with minimal oversight; communication skills (with other care team members, including physicians); positive phone interaction/customer service; and computer skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Compassion, empathy, and dedication to patient care.
Supervisory Responsibility: This position has no supervisory responsibility.
Work Environment:
- May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances.
- May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
- Handles emergency or crisis situations.
- Physical demands also include the ability to lift patients, assist with ambulation, walking, carrying, and pushing.
- OSHA exposure category 1: Position includes tasks that involve exposure to blood, body fluids, tissues.
- Required Personal Protective Equipment as conditions warrant: Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.

Compensation: $25.49 - $40.81 per hour
